About the Campaign
When patients are living with heart failure, it becomes the focus. This means sometimes they need help to manage their symptoms at home.
Our heart failure nurse navigator spends time with each patient focusing on diet, making sure they have and are taking their medications, and helping them learn to screen for symptoms every day.
Simple items like scales and blood pressure cuffs can help patients better manage their health. However, some of our patients can’t afford to purchase these items themselves.
